Dual Eligible D-SNP Plans Help for North Bergen
Some North Bergen residents qualify for both Medicare and Medicaid, which in New Jersey is called NJ FamilyCare. People in this group are known as dual eligible. Having two programs can lower your costs a great deal, but it can also mean two sets of cards, rules and phone numbers.
A Dual Eligible Special Needs Plan, or D-SNP, is a type of Medicare Advantage plan built only for people with both kinds of coverage. It is designed to coordinate your Medicare and Medicaid benefits so they work together, often with a care coordinator who helps arrange services.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
What a D-SNP Can Offer
- Hospital, medical and prescription coverage in one plan
- Low or no copays for many services, because Medicaid helps with costs
- A care coordinator who can help with appointments, home care questions and benefits
- Extra benefits that may include dental, vision, hearing and help with over-the-counter items
Benefits differ by plan, and not every plan is offered in every part of Hudson County. We compare the D-SNPs we carry against your doctors, home care needs and medicines.
New Jersey Plans and Integration
In New Jersey, many D-SNPs are offered by companies that also run NJ FamilyCare managed care plans. When both your Medicare and Medicaid coverage come from the same company, the plans can share information and coordinate care more closely. That matters most for people who use home care, see several specialists or have recently been in the hospital.
Checking Whether You Qualify
You must have Medicare Parts A and B and some level of Medicaid coverage. Some people have full NJ FamilyCare benefits. Others only get help with Medicare costs through a Medicare Savings Program, which we explain on our help paying for Medicare page. Your level of Medicaid affects which D-SNPs you can join.
If you are not sure whether you qualify, we can help you look at your letters. For a full Medicaid application, the Hudson County Board of Social Services handles NJ FamilyCare for aged, blind and disabled residents.
When You Can Change Plans
Dual eligible people have more chances to change plans than most. Rules updated in recent years allow a monthly change into certain integrated plans or back to Original Medicare with a stand-alone drug plan. You can also use the fall Open Enrollment Period. Because the rules are detailed, we confirm your options before any switch.
Talk It Over Before You Decide
A free review with a local, licensed agent costs you nothing and commits you to nothing. We compare the plans we offer against your doctors, medicines and budget, in English or Spanish. Learn more about our agency.

Will I lose my Medicaid if I join a D-SNP?
No. A D-SNP works with your Medicaid. You need to keep your Medicaid active, so watch for renewal letters and answer them on time.
Can I keep my home health aide?
It depends on the plan and on your Medicaid benefits. We check how each plan handles home care before you decide.
Is a D-SNP the only choice for dual eligible people?
No. You can also stay with Original Medicare and a Part D plan, or choose another Medicare Advantage plan. A D-SNP is often a strong fit, but not the only one.
